Accompanying documents for the sample

A filled in appointment to the examination (in English and Russian, in printed form or by hand in block letters) should be filled in by the doctor in the clinic (or the owner of the animal) based on the pet passport.

Photocopies of all completed pages of the pet passport. In the pet passport, all entries must be in English or in Russian and English simultaneously!

A photocopy of the passport of the owner who will transport the animal.
